Specialized Dementia Care
Individualized Care
- Crossroads Homecare treats each client with dementia with respect, empathy, and specialized care that meets their unique needs.
- We develop a customized care plan for the person with dementia, which outlines their specific needs and preferences. This plan will take into account their cognitive abilities, behaviors, and communication styles.
- We establish a routine for the person that includes meals, activities, and rest times.
- We perform a Home Safety Inspection to ensure that the person's home environment is safe and secure, with adequate lighting, non-slip surfaces, and locked doors and windows.
- Our caregivers provide social interaction, mental stimulation, and companionship, and engage the person in meaningful activities.
- Our caregivers are vigilant in observing changes in the person's behavior and reporting them to the appropriate healthcare provider.
Care Plan
Each person affected by dementia is an individual whose care needs are just as unique. All of our services are directed by a Registered Nurse who develops a detailed Plan of Care for each individual that is closely followed and implemented by a Certified Home Health Aide. Our nurse also monitors progress and speaks regularly with the aide, the patient, and the patient's family members to ensure success.
Ongoing Caregiver Training
All caregivers receive regular in-service education on continuing care for persons with Dementia. Our training consists of classroom sessions, required reading, and hands-on training. Caregivers are tested and must exhibit high levels of proficiency to maintain their license and employment.
Monitored by a Registered Nurse
All of the work performed by our caregivers is supervised by a registered nurse to assure safety and satisfaction. We pride ourselves on the excellent level of care we provide.
